The Fort Sill Apache Tribe Community Health Representative (CHR) Program provides services to eligible families in the Apache, Oklahoma Area. The Program initiative is to serve as a link between the clinical setting and the community to facilitate access to services and improve the quality and cultural competence of service delivery. We assist by increasing health knowledge of patients and communities through a broad range of activities such as transportation to health visits, outreach, community education, social support, and advocacy. Patients without other means of transportation will be given assistance to and from the Indian Health Service facilities and other health facilities with a referral from Indian Health Services. The program may also assist with making appointments to the Lawton and Anadarko Indian Health Service Clinics.
